
PCOS and A1c: Why the Scale Stalls While You Improve
Does a new A1c result reflect the changes you made in the last few weeks?
More than most people expect. A1c is often described as a four-month average of blood sugar, which makes a test taken partway through new habits feel meaningless — as if it is mostly reporting on the version of you from before anything changed. In reality the measurement is weighted toward the most recent weeks, so a result drawn two or three months into consistent changes is already carrying a lot of that recent work. A scale that has stopped moving while your waist and your clothes keep changing is a separate story, and in PCOS it is a common and well-recognized one.
Why is A1c not a simple four-month average?
A1c measures the share of your hemoglobin that has glucose stuck to it. Because red blood cells live roughly one hundred to one hundred twenty days, the test looks back over that whole span. What gets lost in the shorthand is that the days inside that window do not count equally.
The kinetic work behind this comes from Tahara and Shima, who tracked how A1c, glycated albumin and fructosamine actually respond when blood sugar is corrected, and mapped the weight each preceding day carries. Their finding was that A1c reflects a weighted mean rather than a simple one, with the heaviest weight on the days closest to the blood draw and a steadily lighter contribution as you go further back. Commonly cited derivations of that model put roughly half the result in the month right before the test, about a quarter in the month before that, and the remaining quarter spread across the earlier weeks.
The practical translation matters if you are being told to disregard a mid-change result. A test drawn eight to twelve weeks into consistent eating, training and sleep changes is substantially a report on those weeks, not on the months preceding them. A later test will give a more settled picture, but the current number is not stale data — it usually already contains more of your progress than "wait, that's all old glucose" implies.
There is a caveat in the other direction, and it is the reason A1c should never be read in isolation. Red blood cell lifespan varies between people, and a modeling study in eLife found a median lifespan of about one hundred days with a meaningful spread around it, enough that individually adjusted A1c differed from the lab value by a clinically relevant margin in some patients. Longer-lived red cells accumulate more glucose and can push A1c up; shorter-lived cells, iron deficiency, recent blood loss and pregnancy itself can pull it down. A1c is a useful trend line, not a precision instrument.
What if the scale has stalled while your measurements keep changing?
This is one of the most common frustrations we hear, and it usually is not a plateau at all. When strength training or higher-intensity exercise starts alongside dietary changes, fat loss and muscle gain can happen at the same time. Skeletal muscle is denser than fat tissue — roughly 1.06 grams per milliliter against about 0.9 — so the same weight of muscle takes up noticeably less room. A body can get visibly smaller and firmer while the scale barely moves.
The exercise literature in PCOS specifically supports watching more than one number. A meta-analysis of exercise and exercise-plus-diet interventions in PCOS found improvements in waist circumference, body fat percentage, insulin resistance markers and cardiorespiratory fitness, with far more modest movement in body weight and BMI. If you only track weight, you will conclude nothing is working while the measures tied to your metabolic and reproductive health are the ones that are actually shifting.
Practical version: keep the tape measure and a set of reference clothes in the picture, take waist measurements at the same time of day and in the same place on your torso, and judge progress over four to six weeks rather than day to day. In the first weeks of a new training program, expect a temporary bump from muscle glycogen and fluid, not fat.
What else can add weight that has nothing to do with fat?
Several things, and they stack on top of real progress rather than cancelling it. Sodium intake, carbohydrate intake, a hard training session, poor sleep, constipation and the luteal phase of your own cycle all move body water by a pound or several across a few days. None of that is fat gain, and none of it is visible on a tape measure the way it is on a scale.
Progesterone deserves a more careful explanation than it usually gets, because bloating and weight fluctuation are among the most commonly reported experiences during luteal support and early pregnancy. The pharmacology is not a simple retention story. Progesterone actually competes with aldosterone at the mineralocorticoid receptor and is natriuretic on its own — in classic human studies it increased sodium excretion, and with continued exposure urinary aldosterone rose several-fold in compensation. That interplay, along with progesterone's effects on smooth muscle and gut transit, is a better explanation for the bloated, heavier feeling than "progesterone makes you retain water." Worth knowing too: synthetic progestins behave differently at that receptor than the progesterone used in fertility care, so experiences with one do not predict the other.
The takeaway is the same either way. Short-term weight noise from hormones, fluid and food volume is not evidence that your metabolic work has failed, and it is not a reason to cut calories further.
Is fasting insulin the same test as A1c or fasting glucose?
No, and this is worth stating plainly because it is a frequent source of false reassurance. Fasting glucose and A1c measure blood sugar. Fasting insulin measures how much insulin your pancreas is producing to hold that blood sugar in range. You can have a perfectly normal glucose and a perfectly normal A1c while insulin runs high behind the scenes — compensated insulin resistance, where the work being done to keep the numbers normal is itself the finding. Fasting insulin paired with fasting glucose is what allows a HOMA-IR calculation, and it is a reasonable, standard test to ask for when insulin resistance is a real question, particularly in PCOS. When should this pattern be looked at properly?
If you have PCOS or irregular cycles, if you have been making genuine changes for a few months without the lab or cycle response you expected, if fatigue after meals is significant, or if fertility treatment has not gone the way the numbers suggested it should, this deserves a real metabolic read rather than another round of "lose weight and try again."
A useful workup looks at fasting glucose and fasting insulin together, A1c as a trend rather than a verdict, lipids, thyroid and inflammatory markers, iron status — which affects A1c interpretation directly — androgens where PCOS is in question, and body composition or waist measurement alongside weight. Ordering and interpreting those tests, and any decision about medication, sits with your physician and your own history.

What should you track over the next month?
Keep it small enough to actually do. Waist measurement once a week at the same time of day. How your reference clothes fit. Energy and hunger in the two hours after your usual breakfast — the single most informative window most people never record. Cycle length and any change in ovulation signs. Training sessions and whether strength is going up, which is the clearest sign muscle is being built while fat comes down. Bring those observations to your next appointment rather than to a search bar. A stalled scale, a mid-change A1c and a normal glucose can all look like nothing is happening and all be wrong about that at the same time. Reading them together is how you find out which dial is actually moving.
