When should you start a prenatal, and does the type of folate matter?

Start at least one month before conception, and ideally about three, because the neural tube closes in the first weeks after conception — often before a positive test even shows up. On folic acid versus methylfolate: the head-to-head research does not support the idea that you need methylfolate because of MTHFR, and either form at a standard prenatal dose is a reasonable choice to confirm with your OB-GYN.

Why "Start Now" Is Real Advice, Not Just Caution

The timing matters more than it might seem. ACOG recommends beginning a prenatal supplement at least one month before conception, and ideally three months out. The reason is specific: the neural tube — which becomes the brain and spinal cord — closes between days 21 and 28 post-conception, which is frequently before pregnancy is even confirmed with a positive test. Starting supplementation only after a positive result means missing a meaningful part of that critical window. Do You Actually Need a Supplement If You Eat Well?

Yes, generally — and this isn't a knock on your diet. ACOG, the USPSTF, and the World Health Organization all recommend prenatal supplementation during preconception and pregnancy specifically because even a genuinely strong diet typically can't close the gap for certain nutrients at the levels pregnancy requires. Folate is the clearest example: the amount needed for meaningful neural tube protection is difficult to reach through food alone, which is exactly why fortification and supplementation exist as public health strategies in the first place. Diet quality still matters enormously alongside supplementation — it isn't an either/or choice — but a prenatal isn't solving a problem that better eating alone can fully replace.

Folic Acid vs. Methylfolate: The Honest Version

This is one of the more genuinely confusing debates in prenatal nutrition, partly because a lot of the content addressing it comes from companies selling one form over the other. Here's what the actual research shows.

Folic acid requires conversion by the MTHFR enzyme before the body can use it, and a real, well-documented phenomenon called unmetabolized folic acid (UMFA) can build up in the blood — this has been shown even at fairly modest supplemental doses combined with folic acid-fortified food intake. The liver-related concerns that circulate around this topic come specifically from animal studies using doses roughly 5 to 10 times the human recommended intake; that's meaningfully different from typical prenatal-dose folic acid, and there's no established human clinical evidence of liver harm at normal supplemental doses. The clinical significance of UMFA itself in humans, at typical doses, remains genuinely unclear in the research — worth knowing rather than either dismissing or overstating.

A 2026 randomized controlled trial comparing methylfolate-based and folic acid-based prenatal multivitamins found the methylfolate group achieved equivalent folate status in both mother and fetus with meaningfully less UMFA buildup, despite the methylfolate formula containing about 30% less total folate content — a real, reasonable point in methylfolate's favor if minimizing UMFA specifically matters to you.

But here's the part that often gets left out of methylfolate marketing: the foundational, large-scale research that established folic acid's neural tube defect prevention — the evidence base ACOG and CDC recommendations are actually built on — used folic acid specifically, not methylfolate. And in a randomized trial specifically comparing methylfolate to folic acid in women with confirmed MTHFR gene variants (C677T and A1298C) and a history of recurrent miscarriage, there was no significant difference in outcomes between the two forms. The idea that MTHFR variant carriers specifically need methylfolate for better outcomes isn't well supported by the one trial designed to test exactly that question directly. The reasonable, non-hyped conclusion: either form, at a standard prenatal dose, is a safe and reasonable choice. Methylfolate has a legitimate rationale if UMFA minimization matters to you personally, but it isn't proven necessary or superior for the outcomes that matter most, and folic acid remains the form with the deepest, most specific evidence behind neural tube defect prevention itself.

What Prenatal Ingredients Are Supposed to Do, vs. What You Actually Get

This is where paying attention to the label matters more than the marketing on the front of the bottle.

Choline (target: 400-450mg) has real biological rationale for fetal brain development and placental function, yet it's one of the most frequently missing or underdosed nutrients in commercial prenatals — a lab analysis testing dozens of prenatal products for actual choline and iodine content found significant gaps between labeled amounts and what was actually measured. Iodine (target: 150-220mcg) supports thyroid function and fetal brain development and is similarly often missing or underdosed, despite mild-to-moderate iodine deficiency being common among women of childbearing age.

Contrast that with high-dose antioxidant combinations (vitamin C and E specifically), which intuitively sound protective — more antioxidants, less oxidative stress, seems logical — but large randomized controlled trials have found no benefit and some signal of potential harm from these specific combinations. "Sounds protective" and "shown to help in trials" aren't the same thing, and it's worth knowing which of your prenatal's ingredients fall into which category. The practical takeaway: don't judge a prenatal by how comprehensive or premium its ingredient list looks. Check specifically for adequate choline and iodine dosing, confirm which folate form and dose it uses, and don't assume a "specialized fertility formula" automatically outperforms a straightforward, well-dosed standard prenatal on the nutrients the evidence actually supports. Start now if you're actively trying — the timing evidence genuinely supports it. Either folic acid or methylfolate, at a normal prenatal dose, is a reasonable and safe choice; the "you need methylfolate" framing is more marketing than settled science.
